What I did..
You've got a tube down your nose or throat. When you start gurgling, the nurse or I would push a suction cather down that tube, and literally vacuum your lungs. If there was mucus that wasn't moving, Rat would take you off the ventilator, squirt salt water down the tube, flush it by giving you large breaths with a resuscitation bag, and then suction again. Sometime the patient would cough it out on his own. Watch out.
