What are these four countries doing on the treatment side that's keeping reported deaths relative to cases so low ??


Maybe they're treating the numbers more than the cases.

Both Saudi Arabia and Pakistan have very young populations. Most of the infected are probably under 50 and aren't going to die. In Turkey those over 65 aren't allowed to go out.
