...then you can proceed If the end objective, a prophylactic functioning as a universal vaccine alternative supported by an RCT, isn't feasible for practical or ethical reasons, then there's no point in doing the preliminaries to an RCT. The most you can do wrt a prophylactic, seems to me, is surveys and epidemiological studies, which would give additional basis for individual patients and doctors choosing to use the drug if they judged it appropriate, which is marginal enhancement of what we have now.
