Zelenko treated high risk patients (old and/or sick) immediately, and low risk - only if they develop symptoms. Overall, as I recall most patients didn’t need treatment. But if they did, he started them ASAP, without waiting for test confirmation, or even for a test if one wasn’t readily available. Incubation period of 6 days for most infected people sounds about right. Some are less resistant - or get a larger load upon infection - than others.
