First of all US with all its resources should be doing better than the third world on that metric.

Secondly,

It is like saying the percentage of people who died when they were shot by a gun is the about the same everywhere for the same gun type.

The difference is that the number of gun homicides per the 100,000 population in the USA is 6 times that of Canada.

Similarly, the covid deaths per 100,000 population in USA is about 6 times that of Germany, when both countries got affected about the same time.
