Election officials do investigate and find very rare cases of voter fraud. Here is WA state we have vote by mail. Voters are sent two envelopes along with a ballot. The ballot is placed in a slightly smaller envelope, which is place in another larger envelope with the person name, address, and any other data. The outside envelope and data are used to verify the elegibility of the voter. Once that is done the inside envelope is processed.
