There is good correlation with chromosomes and that which is checked when filling out birth certificates. There are all sorts of exceptions that can happen however, including a phenomena where and X & Y chromosome combine (actually one tags along on the end of another). This can lead to cases where an individual has XXY or three sex chromosomes. So long as the X is attached to the Y the combo acts in the body mostly as XX (female) but under the influence of hormones during puberty the combined chromosome can break away during cell divisions leading to a free standing X chromosome and the individual begins to manifest male traits after a youth as a female.
