The following link documents cases of death at < 3 ng/ml although more commonly higher and sometimes as high as 11 or 13 ng/ml.

Did you read the clip I included in my post? I didn't evaluate it. Don't have the background. Just clipped and posted the first response that popped up. Looks to be an authentic site.

Nothing there about dying at such a low concentration. Looks like they give people a lot more than that on purpose.

The estimated lethal dose of fentanyl in humans is 2 mg. The recommended serum concentration for analgesia is 1–2 ng/ml and for anaesthesia it is 10–20 ng/ml. Blood concentrations of approximately 7 ng/ml or greater have been associated with fatalities where poly-substance use was involved.
