Breast MR Imaging: Leading Cancer Treatment in the Next Century

The diagnostic capabilities of breast MR imaging are outlined, along
with potential applications of this new technology in clinical
management algorithms. 1. Recent serial section mastectomy
correlations reveal an approximately twofold increase in sensitivity and a threefold increase in specificity of RODEO (rotating delivery of
excitation off resonance) breast MR imaging over those of
mammography. 2. Specific patterns have been demonstrated with
high-resolution RODEO images that can be used to accurately identify
ductal carcinoma in situ (DCIS), determine the probability of an
infiltrating component, and establish the extent of disease. 3. The
Accurate depiction of disease extent by MR imaging can be used to
optimize breast conservation treatment. 4. Since radiation therapy
accounts for a substantial portion of the cost and morbidity associated with breast-conserving surgery, breast MR imaging may aid in the selection of patients likely to benefit from radiation therapy. 5. MR imaging is a highly accurate method for determining chemotherapeutic response and presence of residual disease following induction chemotherapy.
6. Interstitial hyperthermia and cryotherapy have been
used as minimally invasive treatment of cancer. Pathologic correlations with RODEO breast MR imaging have demonstrated a high degree of accuracy in delineating tumor margins and determining extent of disease.
