Till the 19th century, barbers and surgeons were part of the same organization. Surgeons were not respected in the academia.
Look at the Barber's pole outside barber shops...it carries a lot of history.

During medieval times, barbers performed surgery on customers, as well as tooth extractions. The original pole had a brass wash basin at the top (representing the vessel in which leeches were kept) and bottom (representing the basin that received the blood). The pole itself represents the staff that the patient gripped during the procedure to encourage blood flow. [3]

At the Council of Tours in 1163, the clergy was banned from the practice of surgery. [4] From then, physicians were clearly separated from the surgeons and barbers. Later, the role of the barbers was defined by the College de Saint-Côme et Saint-Damien, established by Jean Pitard in Paris circa 1210, [5] as academic surgeons of the long robe and barber surgeons of the short robe.

In Renaissance-era Amsterdam, the surgeons used the colored stripes to indicate that they were prepared to bleed their patients (red), set bones or pull teeth (white), or give a shave if nothing more urgent was needed (blue). [6]

After the formation of the United Barber Surgeon's Company in England, a statute required the barber to use a red and white pole and the surgeon to use a red pole. In France, surgeons used a red pole with a basin attached to identify their offices. Blue often appears on poles in the United States, possibly as a homage to its national colors. Another, more fanciful interpretation of these barber pole colors is that red represents arterial blood, blue is symbolic of venous blood, and white depicts the bandage. [7]

Every one that wants to be called Dr. and is a Ph.D. only proves he/she is not worth of the title and somehow was able to write something once just to get the title.

Real Ph.D.' s can care less how they are called because they know their own worth and no need to display it all the time.

As a side comment - I also take assignments as a substitute teacher for enjoyment mostly as I have no children or relative. So was it today to boys and girls age 11 to13. I give them leadership to ask question and then I explain - Initially wanted to talk about Big Oil and Nigeria.

Today it was hard to believe that I teach about Nigeria NG, Einstein, Chaim Weizmann, Felix Haber, acoustics, radio waves, light spectrum photons electrons and gravitational forces - they asked questions so I needed to explain - was told several times it was very interesting and that is in 3 different classes
