I would submit that one factor, perhaps the main factor, is that a case has never been made for it. Its advocates seem to have one or both of two attitudes. Many simple believe in their hearts that there's a human right to health care. Many take your stated position, that it's backwards not to have universal health care. Those are intuitive positions, not reasoned conclusions. If they come to you naturally, you're in. Otherwise, not so much. I have never seen a serious effort to persuade anyone outside the cohort of the predisposed. Repeating those two mantras louder and louder doesn't do it, at least not in the short term.
