Sounds like this doctor got screwed up first, by the lockdowns in Massachusetts - which forced him to practice telemedicine — and then, because, apparently, Canada doesn’t recognize Telemedicine as a legit type of patient care.

It’s a comedy of [bureaucratic] errors.

I don’t think they would deny him the license because of some subtleties of his training 50 years ago. But, if they can’t recognize Telemedicine as “real” Medicine - apparently by law they can’t license him.

In the US, requirements differ. In NY - it’s simple. Just pay them a certain amount every 2 years - and you’re cool. But many States have requirements, like Continuing Education and also direct patient care.
