Thats what I'd like to know. Did instrumentation on the train or tracks indicate a problem that could have alerted the engineer to stop the train? Was it ignored? Did it malfunction? Thats what needs to be answered. The TMI nuclear accident really was caused by operators ignoring and misinterpreting instrumentation that showed the core was being damaged.
