Either they didn't test it or maybe they did but then there was a last minute change before it went out the door and that wasn't tested.

Or, as in my experience with other things, their testing procedures were inadequate - they should have multiple servers running on several popular brands/types of systems. Probably wouldn't need too many version of Windows since the clients most likely have the latest or nearly the latest Windows iterations.

I'm sure some genius has a thought out SOP for this (?)

Then as you say, a staged rollout so that the screaming is minimal.

Did we forget what a guy named Demming did for Japanese manufacturing quality? That's what he preached – do it right the first time, not after it needs repair. Errors made initially get compounded.
