I've heard, can't verify, that many times when the XRays or MRIs are done, that results are sent overseas to have a radiologist examine them. Not sure if that is true and if may only be for certain medical systems.
Almost all radiology is digitized these days. Unless it is a specialized procedure by an interventional radiologist, most radiology studies are read by radiologists sitting in front of a computer screen. That screen can be in the next room in the hospital or at home/office, especially when emergency studies are done at night. That home or office can be next door or the next state. I don't know what the present rules are for getting paid for reading studies but I believe the physician would have to be licensed in the state where the study was done. I am not sure if a study could be legitimately read in another country unless the physician there was licensed in the state where the study was done.

As to the astronomical price differences, I read an article about 10-15 years ago that most of the difference in price was related to the cost of malpractice. I do think that a big part of the difference is related to malpractice issues but probably not all of it.
