One of the biggest reasons why this happens is that the Pentagon continually tinkers with the specs, requiring the projects to be repeatedly re-engineered. Most recently, this was a factor in the Constellation class frigates being cancelled.

Possible that beginning from an existing design was a mistake? The Navy wanted modifications, but those modifications required further modifications, which in turn required additional modifications. In some ways similar to the issues with the 737 Max, a retrofit to an existing design that (among other things) shifted the flight balance on the air frame making it prone to pitching up.
