They probably aren't fake. Paste pearls are usually white. Those aren't. And they aren't matched, but are different colors, including a black one. Also silver, gold and pink, maybe lavender. Now they may be cultured, black ones often are. But the colors look natural. Dyed or heat-treated pearls are usually more intense.

They are also large and look to be 4 way rollers. Even if all of the pearls are cultured, they are expensive.

Now they could have been a gift. Maybe that is why Bill got divorced. But, as noted, the woman is an adult. Not the adolescents that Trump is pictured with in those files.
