That's why I'm confused about the use of the word cusip with a name change. When the cusip number is changed, you are correct, all shares must be accounted for(I believe shares in name form must be exchanged and those in street form need to be accounted for and then changed over to the new cusip). Only the cusip identifies the stock, the name can change a hundred times. Anyone please correct me if I'm wrong.

This is a guess, but I have a hard time thinking that the SEC has the time to audit outstanding shares of every company doing a reorg. More than likely, since most firms have independant auditors, they let the market take care of that. The BB stocks, who knows. But considering how many there are, there is no way the SEC counts em.
