You ask good questions and I think that SAP gets consulting fees but moreover provides support as a means of selling their product with a guarantee for success.

Products. The SAP Business Framework(tm) includes R/3 Business Components,integration technologies and open interfaces that allow customers to extend their R/3 solutions with independent products from SAP or third parties. The goal of TeamSAP is the successful integration and interoperability of defined components within a customer's R/3 solution.

Longterm consulting may result in more SAP sales of additional independent products than consulting fees. They also mention "third parties" of which I am already aware. Those third parties will take some business from SAP until they can control all aspects of their product with necessary software products. I'm not sure what the "certification" of those third parties products entails, or whether fees are involved. In any case this is not a simple nut to crack, as SAP is a very large and complicated undertaking.
