My understanding is that this is accurate only on a daily basis. EG., if your order is completed in one day but in several parts, you are only charged one commission.

I'm happy to be corrected, though, if my understanding is inaccurate :>)

Depends on the broker. Some will take a GTC and charge you the sum of each day's trades. The first trade would include the base charge, and subsequent days fulfillments would only be the cents-per-share-over-xxx-shares portion of the commission charge.
