Being a member of numerous sites I feel your pain.

What helped me was having a manila file folder and write the necessary info on the inside cover drawing borders around each entry.

i too sometimes forget username/password (especially when they are assigned to me and i had no hand in selecting them). i simply created a file in microsoft word called password. in it i place, for example, the site name, the url, username and password. i've had to go there a time or two <GG>
