The number of new cases being diagnosed on a year to year basis has peaked and may be declining. I think many of the issues that you suggest may be a factor. Some urological academics are patting the profession on the back saying that these numbers show that screening has reduced the risk of prostate cancer and the decline is proof. I think that the number of new cases is mainly a measure of how hard we go looking for them. And these days there is great pressure being placed on the primary care docs to practice "rational medicine" which means eliminating screening for things like prostate cancer. The less you look the less you will find and the less premium revenue you will spend leaving more for executive bonuses and dividends. Don't get me started.
