Sorry for the delay. Been on vacation.

The bug described is not something that could be identified with CodeTest. It is more of the type where a device driver or system dll is removed, something the OS needs to run. I had a similar problem recently where I removed a dll from my NT 4 system and NT wouldn't boot anymore.

I have had a few people use emulators and/or logic analyzers to debug PC's, but it's been a long time. Most of this type of debugging is done with software based debuggers. Companies like Pure Software (now owned by Rational) and NuMega are the leaders here.

Not sure what you mean by score card for efficiency. Care to elaborate.

Problems usually go back to the source, but in embedded, as in most cases, there is always a lot of finger pointing until you can prove who is at fault.

I once did some cosulting for Rockwell. At the time I sold them a CodeTest like tool (SAW Software Analysis Workstation from Northwest Instruments) I had offered some consulting as part of the package, which they declined. Two years later after I had left NW they called wanting help. Made a quick 15K which they were willing to spend because they were having problems at *their* customer site which in this case, was Sprint. So in this case, I was glad their problem rippled back to me.
