Tom: Progression of the naively diagnosed CMV retinitis is not some as progression of the advance retinitis (progression to end stage in last one, and to certain measurable degree at naively diagnosed). So, first prophylactic administration is to delay progression of detected retinitis, and systemic therapy is for non-responded and refractory retinitis as well as one which prophylactic therapy (different in dose and regime from systemic) did not halted.
