Ok this is how some of the more ruthless investors (that are not) MMs short OTC stock... its not exactly ethical, a grey area on being legal. I dont condone, recommend practice or use these methods myself, and am posting this knowledge for educational theories only... (whew hope that enough CYA for myself)

Set up multiple accounts 2 5 10 20 30 accounts (at one time) however many you feel you need or can afford, make sure the broker will allow you to sell in these parameters. But never do this out of your primary stock brokerage account.

A.)You call your broker and tell him you hold the certificate and
would like to sell it. In the meantime you have to get your
shares re-registered because the stock will due in 3 business days.

If the stock is not received within 3 days the broker will grant
you an extension for 2 more days. If the stock is not received during
this extension then the broker will call you and tell you that you
have an additional 5 days to deliver this stock or he will be forced to buy these back at the current market price.

You then tell the broker that you lost the certificate and you then must file a "Lost Certificate" which buys you an additional 30 days.

B) During this time your shares are getting re-registered and you
have just sold short a bulletin board stock. If the stock has fallen
in price at the end of your 40 days that you "sold short" then you
buy it back at the current price and you have made your money.

C) Your account will be put on 90 day restriction and
your broker will not let you sell anything that is not in the account, so you move on to one of the other accounts you have set up and start all over again.

Yeah this is nasty trading, and its ruthless. I dont know if it applies to the situation MDIN is now in, but thats how the average investor can short an OTC right here in the US using a US broker.
