We do not know at this point if it was operator error or some failure of the hardware and software.

This may not be Cisco's fault at all. I have seen operator error all the time in the electronics business.

It could be something simple like someone working on emergency backup power. ( I've seen this at the FAA's regional facilities many times!)

Wonder what the other Carriers and ISP's with Cisco BPX switches are thinking. Is it their network next?

A valid concern. They'll probably increase their capacity to make sure it doesn't happen to them. "Hey, Receiving! Let us know when those shipments from Cisco start arriving!"
