Yes.

1. Web server is frequently down.

2. Inexplicable server errors after building a trade.

3. Flat-out incorrect order execution. I once had an option order go through at the wrong price, and my real-time quote screen (from another vendor) showed that NO options of the type I had allegedly traded were in fact traded that day. Ameritrade STILL has ignored my requests to fix this one, leaving me out $29 and with a screwed-up option position.

But for regular equity orders, I can't recall a time when I successfully entered an order and didn't have it go through correctly.
