There was a crash with a "beta" of W98. It appears someone plugged in a scanner whilst the system was up and running. If that was the case, spikes can be sent from device, especially in absence of good grounding. It is best to plug things in when both computer and peripheral are off.

Unfortunately this occurred during a demonstration of the hot-swap capability of the USB port. The whole idea is that you don't have to power down your system.

Did you test Access 1.0?

It was the buggiest product ever released in the software industry. Nevertheless, it sold well. That's when I knew that Microsoft was unstoppable.
