Another thing from the newsletter: "Eutro is working diligently on the Freedom Medical Holding Group, Inc. distribution and the preferred dividend. However, they have not received the W-9 forms from many of their shareholders. If your stock is held in street name(with your broker) as of January 30, 1998, you need not fill out the forms, but if all or part of your EUTO common and / or preferred stock is held by you, you must provide Eutro with this information in order to process the distribution and dividend. If you were supposed to receive a W-9 form and did not, or if you have any questions concerning the w-9 form, call Eutro and speak with Ms. Donna Grooms, Comptroller.
