I think the real issue here is whether or not Hextend addresses the bleeding issues successfully. I really do not care what the weak link in Hespan is. Hextend appears to have solved it to a large degree.

I got my copy of R.L. Bick's paper from BioTime. They sent me a photocopy of it. I was too lazy to try to find it at the library; besides, it's probably not available out here in Hawaii. I can fax or mail you a copy if you will give me your fax number or mailing address.

The parts of my post in quotes are Bick's words. The rest is mine, including the sentence "The reports of coagulation problems in patients given generic HES-in-saline must then be due, not to HES, but to the saline solution component of these solutions."

It's important to note that Bick didn't study the effects of HES per se on coagulation, he studied the effects of a particular solution containing HES (Hextend). In fact, I don't see how one could study only the effects of HES, since it has to be dissolved in *something* before being put into a person's veins! I think this is the source of much of the confusion. In any case, Bick's results indicate that HES-as-part-of-Hextend doesn't cause any non-dilutional coagulopathy.

Did the paper Anesth Analg v.84 p.206-212 (1997) cite Bick's paper? If it was a thorough review, it should have! ---Mark Voelker
