Previous treatment are NOT less effective for the most difficult to treat cases of ED so your assumption and conclusions are both worng.

There are many thing that health insurance does not cover. Many indemnity plans (unlike HMOs) do not even cover childhood immunizations even though these are well known to prevent illness and even save lives. As as the woman in the NYTimes piece pointed out infertility work up and treatment is very limited in coverage.
