If you call or write, include this information: the name of the person who will own the certificate should be the same as your E*TRADE account the address of the certificate holder should be the same as your E*TRADE account the number of shares that you want certificates for. E*TRADE will determine if the security can be issued in certificate format. If the name and / or address on the certificates will be different from the information on your E*TRADE account, you must mail us a notarized letter signed by all account holders of the E*TRADE account which: states that you give up ownership of the security includes the full name, Social Security number, and date of birth of the new owner if the new owner will be a minor, you must include this information for a custodian, as well.
