The Cliff note version is that MMs buy at the bid and sell at the ask. They commit to buy a given number of shares and to sell a given number of shares.

For issues that are stable, they make/share the spread with another MM. Almost money in the bank, unless they are holding too much before a stock gets hammered.. that's the risk that entitles them to do what they do.
