Now I'm confused. They'd have to accidentally inject the wrong medication, which is similar to the right medication, and then realize somehow that they used the wrong one, and then use phentolamine to counteract the effects? Is that it?

Isn't lidocaine with ephinephrine used by dentists? (Be still, my heart... I have a little pitter pat just thinking of it.)
