they were one-time sales as far as i know. i think the breakdown was 24 fibers for GTE, 12-18 for FRO, 6-12 for WCOM... but not sure if the FRO and WCOM numbers ever came public.

there is some risk for QWST in this strategy, yes, but look at it this way: they were able to recover most of the buildout costs for the network, but they sold only 5% of their maximum fiber plant.

frank, i don't know how FRO, GTE, and WCOM are provisioning their dark fiber. i'm in Hawaii right now and don't have good access to my notes or search tools. i bookmarked your questions and will try to find answers next week.
