According to One investigator in Lancet, bpi is almost totally not seen
by anca to BPI. therefore bpi21 may be quite useful.

From the Lancet>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>
>>". Although the occurrence of autoantibodies against BPI (BPI-ANCA) has been reported in vasculitis, cystic fibrosis, and inflammatory bowel disease2-4 the frequency of BPI-ANCA in patients with gram-negative sepsis has not been recorded so far.............."Missing"................. These antibodies did not recognise the amino-terminal fragment of rBPI21 on testing. Furthermore, analysis of 96 patients with BPI-ANCA revealed only two whose BPI-ANCA recognised rBPI21; however, the underlying disease in these patients was not sepsis but vasculitis and inflammatory bowel disease.5 .....................
Taken together BPI-ANCA were not detected in gram-negative sepsis in noteworthy amounts, and if detected they do not bind to the N-terminal fragment of BPI. Thus the use of rBPI21 in patients with gram-negative sepsis is probably not limited by generation of autoantibodies against BPI.

Moral of the story is anca to bpi "may" not "see" bpi21.
All disclaimers always apply. All IMO,(and others)
