The clinical studies reported flushing and nasal congestion as the most common side effects (if I remember correctly). Considering the nature of the drug, I would suspect that headache, nausea, diarrhea, sweating, visual disturbances could occur. More seriously, hypotension and cardiac arrhythmias could occur (based on side effects reported from intravenous and intramuscular phentolamine). Interestingly, no deaths due to acute toxicity have been reported with IV or IM phentolamine.
