Frank, you must be the only poster here who knows how to use an accent grave. Your posts are not only informative, but worldly as well.

As for PSC measurement of carrier performance, if we think the carriers are slow to grasp new concepts, the regulators are in a different century. I have a friend who is a career regulator in one of the key technology states. He tells me the commissioners have little or no technical knowledge and that the staff analysts, including himself, are accountants. Now I ask, what kind of QoS system will these people be able to use to provide adequate regulation in a packet environment? I have had some asymetrical (simplex?) discussions with them on xDSL spectrum issues, to no avail. Education and restocking are badly needed.

IMO, none of these operational/procedural issues itself is insurmountable. However, the combined effect of all of them is a more delayed roll-out of new services than we all desire.
