How should a "Short Position" be handled with the wash sale rule?

EG. If I traded intc 3 times for a net loss and then immediately went short on intc with in a few days...and stayed short till after DEC. 31st.

While we are on the subject of shorting. How does one report a short sale on Sched D? Do you identify the transaction as a short? What is considered your cost basis if you say shorted INTC from DEC 31st to JAN 2nd? Any other info on shorting as an investor or trader is greatly appreciated!
