250,000 people die of a heart attack before they reach the hospital each year. That is a low percentage of the people who have heart attacks. (1.5 million) Also,according to the Living Heart Diet by Michael Debakey, heart disease or stroke affects one in six men ages 45 to 64.

250,000 / 1.5 million == 16.66%, or 1 in 6. Is 1 in 6 a "low percentage" as in your first argument above or is it the by-implication high number you use in your final argument above?

Also, how many die from their first heart attack after they reach the hospital? My father-in-law died from a first heart attack he suffered while in the hospital.

And "heart disease" is different than "heart attack." "Heart disease" includes the people who are asymptomatic but have plaque build up in their arteries. These people suffering from "heart disease" are the ones that the HMO's would have to pay for preventative care if they were diagnosed before age 65.
