It seems to me if manual backups are required then the technology isn't ready for the small medical office yet. The ONLY cases I know were backups are reliable are large data centers where backups are a major part of somebody's job, or where they're automated.

You might be able to get someone to change RWCD's (or tapes) if you put a scheduler in to run incremental backups automatically each night.
