Buyins are definately based upon when you initiated your position. CNS-Continuos Net Settlement sends the first buyins to naked shorts (fail to deliver) that are out there (mostly MMs) then if that does not satisfy the full buyin, the oldest shorts get bought in.

If your back office is good, they can find another borrow in house or call in a favor from another back office that they've helped out in the past. If your back office is lazy or perhaps automated and has no friends on the Street (especially Italian ones as they own the borrow)you're usually toast and get bought in near the high of the day. An example of a lazy back office is Spear Leads, some of the laziest grade C people around.
