Audio code would be crunched faster than video, so it almost sounds like the DVDROM is reading the audio track wrong. I had a Hollywood and Toshiba 1002 combo and had problems with WIN98 not seeing the drive, even though WIN95 saw it just fine. Ended up going to the 2nd generation drive 1102 and everything is fine.

When I got some supplemental literature
from Sigma, it included a Sony 5x DVD-ROM
drive with REALmagic DVD/MPEG-2 upgrade
for PCs. Maybe that would work for you?
I do not have any Toshiba literature
or product references for separate upgrade.
My PC Cinema is made and sold by
TigerDirect 1-800-888-4437
for $800, including everything
built in, which does happen to have a
Toshiba drive that works perfectly.
One caution though: The TigerDirect
Operating Instruction Manual has some
inaccuracies in it. Be sure to take a bright
light and note connector labels in back
and compare them to the connection drawings.
