this is a message that i received on 9/25/98 from Utah deq:

Class II wells may only inject fluids:

a. which are brought to the surface in connection with conventional oil or natural gas production and may be commingled with wastewaters from gas plants which are an integral part of production operations, unless those waters are classified as a hazardous waste at the time of injection;

b. for enhanced recovery of oil or natural gas; and

c. for storage of hydrocarbons which are liquid at standard temperature and pressure.

Class V wells typically inject non-hazardous, non-radioactive wastes above or into an underground source of drinking water. Class V wells are injection wells not included in Classes I, II, III, or IV.

Regarding questions 2 and 3, I assume you are referring to injection wells in Utah. I hope so, because I can't answer for other states.

Questions 2 and 3: There are 3 Class V permits issued in Utah, none of which are currently operational. Of the three, only one is a commercial facility.

below comment is mine:
"above or into an underground source of drinking water"?????
