I don't think I understand your question. The AMA and various specialty boards govern quality assurance of graduating or existing doc's in this country. It's not a perfect system, but no systems are.

Many potential applicants to medical school are weeded out in undergrad; they simply cannot make the grades/scores. The admission criteria is still very high. I'm afraid in the future this may not be the case.
