The address on the ADOT fact sheet I received says: PMR and Associates 1042 North El Camino Road Suite B-266 Encinitas, California 92024 However, I'm not sure if that address will change soon. The number listed has a 619 area code, but when you call and key in the extension given, you get a recording that says the number for PMR and Associates is now 760-612-3643. That, of course, is a different area code and obviously is not a number within the same building (and I suppose not even the same city).
