No, liner/casing failures are not typical.
If they were, someone would find a better way of getting to the reservoir. Unusual, or unlucky circumstances are responsible for almost all failures of this type.
I'm sure the equipment, whether or not it was rated to handle a well of this magnitude, was in good working order before it went in the hole.
