Cause: A broker will accidentally lean on the sell key while the screen says "AMZN - Sell at Market - 10,000 shares" and due to the auto-repeat function on the keyboard, this order will be executed 1313 times in the matter of 2 minutes. The broker will subsequently be highlighted in a cOUSIN SHORTY newsletter, but will keep his job.
