To add a small suggestion when placing YET another call to any broker that has yet to deliver the following dialogue may come in handy:

My friend still hasn't received his from E-Trade, even though we both requested them over a month ago. Mine were delivered in 17 days. They have deducted a $5 fee from his account, and I told him on Monday to call again. I expect to see him today or tomorrow to find out.
