Andrew, I used the following 2 sites as a reference for the surgery:

cpu-central.com
jps.net

Actually, I found that the soldering part is the most difficult one. Drilling the hole which i first thought would be difficult turned out to be very easy. I did it by hand exactly as described in the refrences above. A bit crucial is the correct choice of the drill, the one mentioned by Kikumaru (0.5mm) is definitely too small. I used a 0.8mm one. If you are living in the inch world you may choose 0.033" (but I haven't checked that out).
The soldering part is really difficult, especially the part where you have to solder a thin wire to the B75 contact. This point turned out to be very critical later when I pulled the CPU several times in and out for testing purpose. The wire to contact B75 always tends to get lose. Of course, everything is more easier if you have the right tools for SMD technique. But I did everything at home where I only have standard tools. All in all I strongly suggest to follow the steps mentioned in the second link.
An unexpected problem was NT. I thought NT would switch automatically to the multiprocessor kernel, but found out later that you have to install a special multiprocessor kernel. There is a small untiliy available by MS which allows you to switch to the multiprocessor kernel, but a quick search on the net revealed that people have a lot probs with that tool (i think the first version had an error). Anyway, I finally made a new install of NT and everything works perfect now for a couple of weeks.
